Hospital preference (optional)

You choose the hospital. We don't recommend specific hospitals or doctors.

Companion fee only, not hospital costs. You pay hospital, lab, pharmacy and transport costs directly to the provider.

Optional, logistics only — e.g., "staying near Guomao", "arriving from the airport", "prefer a female companion". Please do not include any health information (symptoms, conditions, departments, test results, medication). If we need health-related details to plan your visit, we'll ask you separately by WeChat or phone, with your separate consent.

Before you submit

We use the information in this form only to reply to your request and arrange your hospital companion service. This form is hosted by service providers in the United States (listed in our Privacy Policy), so submitting it means your form information is transferred overseas. We never ask for health information here. By default, we keep form records for 90 days after your service. You can ask us to delete your information at any time. Prefer not to use the form?
